Adult Bleeding Disorders Program

Before your appointment, please sign up for Thrive, a web application we use to prepare us for your visit. You can also use Thrive us information electronically. You will receive a separate email with a link to sign up for Thrive.

Once you are registered, please fill out a short pre-clinic questionnaire(s). Please fill out the questionnaire(s) 2-3 weeks before your appointment. We will use your up-to-date information to make your clinic visit better.

If you are on the home factor infusion program, please have all of your infusion records up-to-date via iCHIP at least a week before your appointment. You may request an account and register for iCHIP by contacting the clinic at 604-806-8855, or by email.

In the meantime, review these links to introduce you to the system, connect to the log-in page, and complete the consent form that the staff will use to enroll you.

If you are travelling to the clinic, you may be eligible for some support programs to help with the cost of travel.

You may be able to get reimbursed (get your money back) for ferry or flight costs through the Travel Assistance Program of BC (TAP)  If you meet the eligibility requirements listed on the TAP website, request a TAP form from the Adult Bleeding Disorders Team.

If your round-trip travel to clinic is more than 200 kilometres total, the BC Chapter of the Hemophilia Society may be able to reimburse you (give you money back) for some travel costs. You will require the following to be considered for funding:

• Membership with the BC Chapter of the Hemophilia Society. Complete and submit the membership form.
• A letter confirming you are registered with the Adult Bleeding Disorders Program of BC. You may request this letter from the Adult Bleeding Disorders Team.
• A funding request form is to be completed and submitted after your appointment. You may request this form from the Adult Bleeding Disorders Team.

You may have a clinic appointment scheduled via Zoom or telephone. Our office will assist you in setting this up prior to your appointment. 

To prepare for your virtual appointment:

• Learn how to set up your Zoom appointment
• Read the Providence Virtual Health Privacy Guidelines
• Prior to your appointment please register for Thrive
• Have your goals for this appointment in front of you
• Make sure you know your current weight
• Bring the things you usually bring to an appointment (for example your medication list, braces, gait aid, etc.)

For more support with Virtual Health please contact the Patient Virtual Healthcare Tech Support desk at 1-844-442-4433 (Monday-Friday, 7:00 a.m. to 5:00 p.m.). Providence created the Support desk to offer telephone support for patients using the Zoom for Healthcare platform for their Virtual Health Visits. The team can help you install or access the Virtual Health Visit tool, set up the camera and microphone, or test the connection.
