Being diagnosed with diabetes can be an overwhelming experience for you and your loved ones. We are here to offer you the knowledge and tools you need to successfully self-manage your diabetes.

How we help you self-manage your diabetes

We are here to help you self-manage your diabetes by providing support and education on:

  • Meal planning
  • Physical Activity
  • Medication management
  • Blood glucose control
  • SMART (Specific, Measurable, Achievable, Relevant and Time-Bound) Goal Setting

Our diabetes education & support programs

Our programs are made up of group classes, with some individual appointments. They are taught by a Registered Nurse and/or a Registered Dietitian and can be in-person or online. Our education programs are interactive and support you towards your goals. A family member is welcome to come with you if space is available.

You will need a referral from your family doctor for most of the programs offered.
