Diabetes Health Centre

The Diabetes Health Clinic is located at St. Paul’s Hospital at 1081 Burrard St, Vancouver, BC. Remember to include about 30 minutes more time before your appointment to get from your car or transit to the clinic area.

In-person group education sessions are held at:

Transportation & parking

Please bring the following to your appointment:

• Your BC Services Card
• A list of all your current medications and how you take them
• Your blood sugar meter and a record of your recent blood sugars
• Your insulin pump and/or your continuous glucose monitor sensor downloads
• A record of what you have eaten in the past 3 days
• A written summary of your medical history, if you have one
• Family members and support persons
• Questions and concerns that you would like addressed by the team

All of Providence Health Care’s hospitals and residences are scent-free environments. Please do not wear scented products.

If you need a sign language interpreter (at no cost to you), please let us know in advance.

If you need to change or cancel your appointment, please call the Clinic at 604-806-8357 within 48 hours of your appointment. This allows us time to schedule other patients.
