Independent Dialysis

Ongoing follow up

If you are on peritoneal dialysis, you will come to the clinic every three months to meet with the care team. The team includes nurses, nephrologists (kidney specialists), patient educators, dietitians, social workers, pharmacists and physiotherapists.

If you are on home hemodialysis, you will come in for clinic visits every one to three months.
