What You Will Do

Summary

You will play a vital role as part of a multidisciplinary team that assists older adults with their acute health care needs, including providing care to those who have experienced significant functional decline in a short time frame, or who are experiencing multiple complex medical and/or psychiatric problems. In some situations, they may require rehabilitation/reactivation to return home. The Acute Care Medicine Unit for Older Adults is an expanded service of Geriatric Medicine on 9C/D. The care model meets the growing needs of care for the older adult population with acute illnesses.

Your responsibilities will include:

  • Providing professional guidance to other healthcare professionals, working autonomously and collaboratively within an interdisciplinary healthcare team.
  • Making referrals to specialist physicians and other resources as necessary.
  • Collaborating with patients and healthcare professionals to identify trends and patterns that impact the community’s health.
  • Participating in evidence-based strategies to enhance healthcare services.
  • Contributing to policy-making activities influencing healthcare practices.

What You Bring

Education:

  • Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals.
  • Successful completion or awaiting results of the Objective Structured Clinical Examination (OSCE).
  • Completion of the Basic Cardiac Life Support Course, Level C.

  • A competitive salary: $74.45-$81.90 per hour, depending on NP experience. Reimbursement is also available upon successful completion of the Objective Structured Clinical Examination (OSCE) for NPs who have not yet completed the exam.

Your Day to Day:

  • Diagnoses and treats patients for undifferentiated diseases, disorders, and conditions within scope of practice; provides first-line care in emergencies.
  • Monitors ongoing care, orders appropriate diagnostic investigations, interprets reports, and analyzes information to plan treatment.
  • Establishes priorities for health management; provides follow-up treatment; communicates findings, diagnoses, and priorities to patients and families; supports and counsels patients in their responses to diseases and conditions.
  • Collaborates with medical staff colleagues and healthcare team to determine appropriate resources and develop treatment plans; may admit and discharge patients according to medical staff bylaws.
  • Prescribes drugs within statutory and regulatory standards for Nurse Practitioners and PHC medical staff rules.
  • Provides mentorship and education related to patient care to registered nurses, particularly junior RNs and LPNs.
  • Participates in research and quality improvement initiatives to enhance patient care and organizational quality.
  • Implements screening and health promotion activities for at-risk populations, patients, and families.
  • Educates nurse practitioner students and other healthcare professionals through preceptorship, role modeling, and knowledge exchange.
  • Develops, implements, and evaluates policies and procedures related to medical staff quality, advanced practice nursing, interdisciplinary care, and health system practices.
  • Completes Encounter Records as required by the Ministry of Health to evaluate Nurse Practitioner implementation.
