Post-Heart Transplant Clinic

The clinic will follow your health closely, especially through the first year after your transplant. We will then see you at least once or twice annually thereafter, unless you transfer to one of the regional transplant programs.

We will also provide you with information on how to contact the clinic or your nurse, and when you need to reach out to them to report any worsening post-transplant related symptoms.
