Kidney Care Clinics

You can be referred to the kidney care clinic at any stage of kidney disease. Our team will assess you and provide your ongoing kidney care.

The clinic will help you:

• Better understand your kidney condition.
• Keep your kidneys healthy or stable for as long as possible.
• Understand the best diet to keep your kidneys healthy.
• Learn about lifestyle changes that are best for your kidneys.
• Help you and your loved ones cope with kidney disease.
• Learn about options if your kidney function declines.

If you have kidney disease and diabetes, you can have your care in one coordinated clinic visit with the Integrated Care Clinic. Ask your current kidney or diabetes specialist for a referral. Our team will assess you and provide your ongoing kidney and diabetes care.

Our team of nurses, pharmacists, dietitians, and social workers have training in both kidney and diabetes care. You will also be seen by a kidney specialist (nephrologist) and diabetes specialist (endocrinologist) at the visit. The team will help you understand how to manage your diabetes and your kidney disease.

The clinic will help you:

• Control your diabetes with confidence.
• Understand the best diet for both your diabetes and your kidney disease – tailoring a plan specific to your needs and preferences.
• Help you and loved ones cope with both diseases.
• Learn about options if you kidney function declines.

If you have been diagnosed with Polycystic Kidney Disease (PKD), you will be seen at this clinic. You may also be referred if your doctor suspects a cystic kidney condition as the clinic team can help with diagnosis.

Our team includes nurses, pharmacists, dietitians, social workers and kidney specialists (nephrologists). We are experienced in diagnosing and treating polycystic kidney disease.

The clinic will work with you to develop a treatment plan that is best for you. They will also offer ongoing follow-up and support.

If you have been diagnosed with one of a group of kidney conditions commonly referred to as a “glomerulonephritis”, you will be seen at this clinic. These conditions may include lupus, vasculitis or other conditions.

The GN clinic will give you additional advice and support. You will continue to see your original kidney specialist (nephrologist). In some cases, you may have only one visit to confirm a diagnosis or treatment plan. In others, you may visit the clinic several times so that the team can assess your response to therapy.

This clinic accepts referrals for patients living in BC and Yukon. We offer virtual visits as required.

You will be referred to this clinic if you have kidney disease or a kidney transplant and you’re pregnant or considering getting pregnant. Our specialists will assess your risk and answer your questions as you plan for a pregnancy. They will also support you throughout your pregnancy.

The clinic will help you:

• Learn about how a pregnancy might affect your kidney condition.
• Understand the potential risks and outcomes of a pregnancy based on your specific kidney condition.
• Optimize your health before and during pregnancy.
• Develop a tailored care plan for during and after your pregnancy. This will involve working with your other health providers (e.g. nephrologist, family doctor, midwife, obstetrician).

This clinic accepts referrals for patients living in BC and Yukon. We offer virtual visits as required.

Your kidney disease might have a genetic cause. To confirm, your kidney specialist (nephrologist) will refer you to the Kidney Genetics Clinic for an assessment. We specialize in the evaluation, diagnosis and treatment of genetic kidney diseases.

At your first visit, the team will review your kidney condition and family history. They will also discuss the type of genetic testing that might be available for you. If appropriate, the team will arrange genetic testing. Once the results are back, the clinic team will meet with you to explain the results and discuss a treatment plan.

Your kidney specialist (nephrologist) will continue to manage your ongoing kidney care.

This clinic accepts referrals for patients living in BC and Yukon. We offer virtual visits as required.

If you are already followed by a nephrologist, they will work with you to decide on the best time to transition you to the kidney care clinic or integrated care clinic.

If you do not have a nephrologist, ask your primary care provider to refer you to a nephrologist in your area. The nephrologist will arrange transition to the KCC/ICC as appropriate.

Referrals are accepted from primary care physicians, nephrologists, obstetricians, internal medicine doctors and maternal fetal medicine specialists.
