Preparing for your visit

What to bring:

  • BC Services Card
  • summary of your medical history
  • Family history of cardiac illnesses
  • List of current medications, including non-prescription medications
  • Family History (Genetic) questionnaire – if you have not already returned this form to the Genetic Counselor
  • Names of all health care providers that you see
  • Questions and concerns that you would like addressed by the team

Transition to Adult Care Services

The Scotiabank Youth Transition Program (SYTP) is a program offered at St. Paul's Hospital to help support young people (ages 16 – 25 years) and their families with the transition from pediatric to adult care. The SYTP provides information and resources to help young people and their families as they navigate their transition into adult care services. Transportation & parking

Remember to allow time (approximately 30 minutes) before your appointment to get from your car or transit to the clinic area. If you are new to the clinic and need directions when you arrive at St. Paul’s Hospital, ask a volunteer or reception for directions to our front door. Please remember the name of the clinic to get the best assistance.
