Preparing for your visit

You must be referred by a primary care physician, nurse practitioner, internist or a cardiologist to book an appointment. (Referral form below).

Getting ready

What to bring:

  • BC Services Card
  • All your medications with you in a bag so we can see exactly what you’re taking
  • Your medication list, if you have one
  • Summary of your medical history, if you have one
  • A list of your doctors
  • A record of your weight for the last month (if you have it)
  • A record of your blood pressure and blood sugar levels for the last month (if you have them)

Cancellation policy

To cancel or change your appointment, please call the clinic within 48 hours of your appointment. This allows us time to schedule other patients.

If you feel very sick and your symptoms worsen, please see your primary care practitioner or visit the Emergency Department.

Transportation & parking

Please allow about 30 minutes before your appointment to get from your car or transit to the clinic area. If you are new to the clinic and need directions, when you arrive at St. Paul’s Hospital, ask a volunteer or reception for directions to our front door. Please remember the name of the clinic to get the best help.
