Early Intervention Memory Clinic

Services available through this clinic

Our team includes geriatricians and occupational therapists. We make referrals to physiotherapists, dietitians, social workers, and neuropsychologists as needed. We work within the Older Adult Outpatient Clinic at St. Paul’s Hospital.

After we receive your referral, our occupational therapist will call you to review:

  • Your hopes for the program
  • Your health history
  • Your screening questionnaire (concerns about brain health and memory and thinking)
  • What matters most to you

The following services may be offered based on your needs:

  • Functional and cognitive assessment by an occupational therapist
  • Targeted medical assessment by a geriatrician. This may include sharing recommendations with your family physician/nurse practitioner. They may request additional medical tests & imaging
  • Individual occupational therapy for cognitive strategies and brain health habits
  • Online or in-person brain health education and behaviour change groups for person with cognitive change and care partner
  • Transfer to Older Adult Outpatient Clinic for more comprehensive geriatrician assessment as needed
