Preparing for your visit
Questionnaire
Before your first visit, you need to fill out a questionnaire to tell us about your conditions.
Questionnaires are available in online and paper format. You can ask our office assistants to mail them to you ahead of time. We prefer to have them filled out online.

Communicate any preferences
We are a teaching site for the University of British Columbia. Medical students, residents and fellows train with us.
Please tell us before your visit if you do not want trainees to be part of your clinical care.
Surgery
If you need surgery, it will take place either at St. Paul's Hospital or BC Women's Hospital and Health Centre.
