Person & Family Centred Care

Person and Family Centered Care (PFCC) is a foundational principle of PHC's Mission: Forward Strategic Plan. PFCC is an approach to the planning, delivery and evaluation of health care, done in partnership with health care providers, patients, residents and families.

We strive to provide the best possible care experience to our patients, residents and families by enhancing engagement with them across all levels in the organization. At Providence, we incorporate PFCC practice through education, various research and quality improvement projects.

PFCC practice is guided by four core principles:

  1. Patients, residents and families are treated with respect & dignity
  2. Patients, residents and families are invited to participate in clinical decision making at the level they choose
  3. Patients, residents and families are invited to collaborate with us as partners throughout the organizational levels
  4. Patients, residents and families have timely access to information so they can be involved in their care
